Assumption of risk and release

I understand that baseball, softball, athletic training, use of batting cages and pitching machines, strength and agility work, lessons, practices, camps, tryouts, rentals, and related activities involve risks including falls, collisions, being struck by balls or equipment, equipment failure, and serious injury.

I voluntarily authorize the athlete named above to participate. To the fullest extent permitted by law, I release and hold harmless Top Prospects Training Facility, Prospects Baseball, their owners, coaches, employees, contractors, and facility partners from claims arising from ordinary negligence connected with participation or facility use. This release does not apply where prohibited by law.

I authorize reasonable emergency care when I cannot be reached and accept responsibility for resulting costs. I agree to follow facility rules, supervise minor children when required, and immediately report unsafe conditions or injuries.
